Drug Target Encyclopedia Series (27) —KLRK1

KLRK1, better known as NKG2D, is an activating immunoreceptor expressed on NK cells, CD8⁺ T cells, γδ T cells, and some NKT cells.
It functions as a stress-surveillance receptor, recognizing ligands induced by cellular transformation, infection, or DNA damage.

In immuno-oncology and immune modulation, KLRK1 represents a core axis of innate–adaptive immune activation and a high-value target for restoring anti-tumor cytotoxicity.

Where KLRK1 matters biologically

NKG2D binds to stress-induced ligands such as MICA, MICB, and ULBP family proteins, which are frequently upregulated in:

• Solid tumors (lung, colorectal, ovarian, melanoma, HCC)
• Hematological malignancies
• Virus-infected or genomically unstable cells

Loss, shedding, or downregulation of NKG2D ligands is a key immune-evasion mechanism, strongly associated with tumor progression and poor prognosis.

Disease & drug development value

From a drug-discovery perspective, KLRK1 sits at the intersection of:

• Innate immune activation
• Tumor immune surveillance
• Resistance to checkpoint blockade
• NK- and T-cell–based cell therapies

Enhancing or restoring NKG2D signaling can directly boost cytotoxic lymphocyte killing, making it a strategic target beyond classical checkpoints.

Emerging therapeutic strategies targeting KLRK1

🔹 NKG2D-based CAR platforms
NKG2D CAR-T and CAR-NK cells exploit ligand overexpression across multiple tumor types, enabling broad-spectrum tumor recognition independent of HLA.

🔹 Bispecific & multispecific antibodies
Antibodies linking NKG2D ligands to CD3 or NK receptors aim to redirect immune effector cells toward stressed tumor cells.

🔹 Ligand stabilization & shedding inhibition
Blocking MICA/MICB shedding restores surface ligand density, re-sensitizing tumors to NKG2D-mediated killing.

🔹 mRNA & transient expression systems
mRNA tools are increasingly used to:
• Express NKG2D receptors or ligands transiently
• Engineer NK and T cells without permanent genomic modification
• Rapidly validate CAR and bispecific designs
• Model stress-ligand dynamics in tumor cells

 Key challenges

• Tumor-mediated NKG2D ligand shedding
• Chronic stimulation leading to receptor downregulation
• On-target activity in inflammatory settings
• Translating potent cytotoxicity into durable clinical responses

Current R&D focus areas

• Next-gen NKG2D CAR-NK therapies
• Combination with PD-1 / PD-L1 blockade
• Targeting ligand biology as a biomarker strategy
• mRNA-driven immune cell engineering
• Overcoming tumor-induced NKG2D desensitization
