Drug Target Encyclopedia Series (28) —FOLR1

FOLR1 (Folate Receptor Alpha, FRα) is a GPI-anchored membrane protein that mediates high-affinity folate uptake.Under physiological conditions, FOLR1 expression is low and polarized in normal epithelial tissues, but it is markedly upregulated in multiple solid tumors, making it a highly differentiated and clinically validated oncology target.

What is FOLR1?

  • High-affinity folate-binding receptor
  • Anchored on the cell surface via GPI linkage
  • Enables folate internalization through receptor-mediated endocytosis
  • Expression pattern: tumor-enriched, normal tissue–restricted

This sharp tumor–normal contrast is the core reason FOLR1 has become a premium ADC and targeted delivery receptor.

Disease & Drug Value

FOLR1 is highly expressed in:

  • Ovarian cancer (high-grade serous, strongest clinical validation)
  • Endometrial cancer
  • Non–small cell lung cancer (adenocarcinoma)
  • Triple-negative breast cancer (subset)
  • Certain renal and gastric cancers

High FOLR1 expression correlates with:

  • Aggressive tumor biology
  • Poor prognosis
  • Strong suitability for targeted payload delivery

👉 Clinically, FOLR1 is one of the most successful solid tumor surface antigens beyond HER2.

Emerging Therapeutic Strategies

🧪 Antibody–Drug Conjugates (ADCs)
The dominant modality for FOLR1:

  • Efficient receptor internalization
  • High tumor selectivity
  • Proven clinical responses in ovarian cancer

🧪 Bispecific antibodies

  • FOLR1 × CD3 approaches to redirect T cells
  • Requires tight control of on-target toxicity

🧪 Targeted imaging & radioligands

  • Companion diagnostics
  • Patient stratification for ADC eligibility

🧪 mRNA-based research tools

  • Rapid validation of FOLR1 expression constructs
  • ADC internalization and trafficking studies
  • CAR and bispecific design screening
  • Safer, transient expression for early discovery

Key Challenges

  • Managing on-target off-tumor toxicity
  • Tumor heterogeneity of FOLR1 expression
  • ADC payload optimization (potency vs safety)
  • Patient selection and biomarker cutoffs

Industry Takeaway

FOLR1 represents a gold-standard membrane target for solid tumors:

✔ Clear expression differential

✔ Strong internalization biology

✔ ADC-first commercial validation

✔ Expanding into bispecifics & imaging

As the ADC field matures, FOLR1 continues to define how “good targets” should look—biologically, clinically, and commercially.
