FGFR4 is a transmembrane receptor tyrosine kinase in the FGFR family (FGFR1–4) and plays a central role in cell proliferation, metabolism regulation, tissue repair, and tumor progression.
In oncology, FGFR4 has gained strong interest due to its oncogenic signaling, tumor-specific alterations, and its involvement in aggressive, therapy-resistant cancers.
FGFR4 is highly expressed or activated in:
Hepatocellular carcinoma (HCC) — especially in FGF19–FGFR4 amplified tumors
- Rhabdomyosarcoma
- Colorectal cancer
- Breast cancer
- Prostate cancer
- Cholangiocarcinoma
- Several FGF19-driven solid tumors
FGFR4 activation promotes cancer cell survival, metastasis, and metabolic rewiring, making it a highly druggable target across precision oncology programs.
Targeting FGFR4 in oncology & immunotherapy
- Small-molecule FGFR4 inhibitors (FGFR4-selective TKIs)
Selective inhibitors—such as those blocking the FGF19–FGFR4 axis—have shown promise in FGF19-amplified HCC and other FGFR4-dependent cancers.
- Monoclonal antibodies & ligand traps
FGFR4-directed antibodies and FGF19-neutralizing mAbs aim to disrupt ligand–receptor binding while reducing off-target effects.
- Antibody–drug conjugates (ADCs)
FGFR4-targeted ADCs are being investigated for tumors with high FGFR4 surface density, providing selective cytotoxic delivery.
- CAR-T & CAR-NK cell therapies
FGFR4 CAR platforms (early-stage research) are being evaluated for:
- Rhabdomyosarcoma
- HCC
- Other FGFR4-high solid tumors
Focus areas include tumor specificity, antigen density, and reducing hepatotoxicity.
- mRNA & RNA-based technologies
mRNA tools are increasingly used to:
- Modulate FGFR4 signaling
- Study FGF19–FGFR4–mediated metabolic pathways
- Engineer immune cells for FGFR4-targeted applications
- Build rapid screening models for inhibitor validation
Key challenges
- FGFR4 expression in normal liver tissue → risk of hepatotoxicity
- Resistance driven by pathway cross-talk and adaptive signaling
- Heterogeneous expression across solid tumors
- Lack of robust biomarkers to identify FGFR4-dependent disease
Current research directions
- Next-generation irreversible FGFR4 inhibitors
- Combination therapy with PD-1/PD-L1 inhibitors, VEGF blockers, or metabolic modulators
- High-selectivity ADC development
- mRNA-based pathway interrogation for safer target validation
- FGF19/FGFR4 axis blockade in HCC precision medicine
