In vivo CAR-T Insights Series (1) — What is In vivo CAR-T?

What is In vivo CAR-T? How does it differ from traditional ex vivo CAR-T?

Traditional ex vivo CAR-T workflow: Collect patient’s T cells →Genetically modify them outside the body → Infuse them back into the patient.

While this approach has shown remarkable clinical success, it involves complex manufacturing, high costs, and long turnaround times.

In Vivo CAR-T — a new paradigm:

Instead of modifying T cells in a lab, in vivo CAR-T delivers genetic instructions directly inside the patient’s body, turning their own T cells into CAR-T cells on-site.

Key advantages:

1. Streamlined process – No cell collection or external manufacturing.

2. Lower costs – Eliminates expensive cell culture facilities.

3. Scalable – Potential for broader patient accessibility.

Challenges to address:

Delivery efficiency – Getting genetic material precisely to T cells.

Specificity – Avoiding off-target cell modification.

Safety – Minimizing immune-related toxicities.
