Drug Target Encyclopedia Series (18) —GPRC5D

GPRC5D (G protein–coupled receptor, class C, group 5, member D) is an orphan receptor highly expressed in multiple myeloma (MM) cells but minimally in normal tissues, making it a promising immuno-oncology target for novel therapies.

GPRC5D is implicated in:
• Multiple Myeloma (relapsed/refractory MM)
• Extramedullary plasmacytomas
• Bone marrow–associated malignancies

Targeting GPRC5D in therapy:

  • CAR-T cell therapy – Engineered T cells show strong anti-myeloma activity.
  • Bispecific T-cell engagers (BiTEs) – Redirect T cells to attack GPRC5D+ plasma cells.
  • Antibody-drug conjugates (ADCs) – Selective cytotoxic payload delivery to myeloma cells.

Key challenges include:
• Understanding resistance mechanisms in GPRC5D-targeted therapies
• Managing on-target/off-tumor effects (e.g., keratinized tissue expression)
• Need for combinational strategies with BCMA-targeted therapies

🔬 Ongoing research is rapidly advancing GPRC5D-targeted immunotherapies, with encouraging results in clinical trials for relapsed or refractory multiple myeloma.
