B-cell maturation antigen (BCMA) is a key member of the tumor necrosis factor receptor superfamily (TNFRSF17), primarily expressed on mature B cells and plasma cells. Its critical role in B-cell survival and differentiation makes it an essential target for treating multiple myeloma (MM) and other B-cell disorders. Unlike other B-cell markers, BCMA is absent on naïve and memory B cells, as well as hematopoietic stem cells, allowing for selective depletion of malignant plasma cells while preserving normal B-cell populations.
BCMA-targeted therapies have revolutionized multiple myeloma treatment, leading to the development of antibody-drug conjugates (ADCs), bispecific antibodies (BsAbs), and chimeric antigen receptor (CAR) T-cell therapies.
FDA-approved BCMA therapies include:
Belantamab mafodotin (Blenrep) – An ADC that delivers a cytotoxic payload to BCMA-expressing cells.
Teclistamab (Tecvayli) – A bispecific antibody that engages both BCMA and CD3 to recruit T cells for targeted killing.
Idecabtagene vicleucel (Abecma) & Ciltacabtagene autoleucel (Carvykti) – CAR-T therapies that harness the power of engineered T cells to eradicate myeloma cells.
Beyond multiple myeloma, BCMA-based therapies are being explored in systemic lupus erythematosus (SLE) and other autoimmune conditions, aiming to restore immune balance by selectively targeting pathogenic plasma cells.
The next generation of BCMA-targeted treatments focuses on overcoming resistance, enhancing durability, and reducing toxicity, ensuring better clinical outcomes for patients.
